The Baseline Visit is a confidential virtual appointment with a Curative clinician, scheduled within the first 120 days of your plan enrollment. Completing it ensures you pay $0 out-of-pocket for in-network services and preferred prescriptions for the entire year. It’s not a test or evaluation that affects your premiums or coverage renewal, but a chance to discuss your health goals and understand your benefits​.

If you don't complete your Baseline Visit within 120 days, you'll incur higher costs: a $5,000 individual or $10,000 family deductible applies, along with copays and coinsurance for most services​.

You can easily find in-network providers by visiting www.curative.com/get-care or calling Curative Member Services at (855) 428-7284. Additionally, a Care Navigator will assist you in confirming that your preferred providers and pharmacies are in-network during your Baseline Visit​​.

Some services, like inpatient hospital stays, outpatient surgeries, advanced imaging (MRI, CT, PET scans), and specialty medications, require prior authorization. Curative Care Navigators can help you determine when authorization is necessary and assist with the process​​.

Dependents under 18 are automatically eligible for $0 copay and deductible if at least one parent completes their Baseline Visit. Dependents 18 and older must complete their own Baseline Visit to qualify for these benefits​.
